The name of the 57th chapter of Uttaratantra of Sushruta Samhita is Arokaka Pratisedha. This chapter deals with the treatment of anorexia.

We will now describe Aroka Pratisedha – the treatment of anorexia (loss of taste), as prescribed by Venerable Dhanvantari.
Diagnosis – Causes and Pathogenesis
दोशैः अभिब्द सह च चित्विपर्यायच्च भक्तायनेशु हर्दी चावताते प्रगाधम् |

Arokāka manifests as five types – three types due to the aggravated dosha of each individual, one type due to aggravation of all three doshas and the fifth type due to disorders of the mind (causative factors like too much Kama, Soka, fear etc.).
In this way the defects get accumulated in large quantities and spread to the areas of food (taste) and heart. In this condition the person is not able to taste the food present in the mouth. Doctors call this disease Bhaktoghata (Arokaka), this disease has been described as of five types.

Arochka is also produced by the following factors –
– lust
– Sadness
– Fear
– Worry
– Dirty things like seeing scary or disgusting things, unpleasant smells etc.

For Vaataja Arochka –
Vomiting – Vomiting should be induced first by giving decoction of Kashaya – Vacha – Acorus calamus.
After this, the patient is given to lick the powder of the following herbs mixed with either oil, sura – beer or warm water –
– Krishna
– Vidanga
– yava bhasma
– harenu
– colorful
– Rasna
– Ila
– hingu
– Lovenottam
– nagara
पित्ते गुदाम्बमधुरायर्वमनम् मित्रं स्नेह ससैंधवसितमधुसरपिरिष्ठाह ||7||
For pittaja arochaka –
Vomiting – The patient should be given jaggery water and sweet tasting herbs (Madhuca etc.) and induce vomiting.
Later, Sneha should be given by mixing oil or ghee with the following ingredients –
– Saindhawa
– Sita
– honey
– snake
Nimbambuvamitvatah kafje’nupanam rajadrumambu madhuna tu sadipyakam syat.

In Kafja Arochka –
Vomiting – Nimba – Vomiting should be induced by consuming decoction of Azadirachta indica.
Later, the patient is given a decoction of Rajadruma mixed with honey and Dipyaka to drink.
Alternatively, a powder prescribed for Vataja Arochka may be prescribed.
All treatments for Tridoshaja Arochka should be adopted appropriately.
द्रक्षपतोलविद्वेत्रकरिरिम्बमूरवाभ्याक्षबद्रमलकेंद्रवृक्षैः |
बीज़ कर्जन्रपव्रक्षभावैश्च पिष्टेर्लेहं पचेत सुरभिमुत्रयुतं यतावत् ||य||
Mustam Vachanam Trikatukam Rajnidvayam Cha Bhargin Cha Kusthmath Nirdhanim Cha Pishtva |
वृत्रेऽविजे द्विर्दमुत्रायूते पचेद्व पाठां टुगामतिविषां रजनिन् च मुख्यम् ||10||
Mandukimarkamritam cha salangalakhyaan urine pachettu mahishasya vidhanvidva.
Atanna shanti chaturo lihastu lehan gulmaruchishwasankanthahridamayascha ||11||
Drakshadi Leha –
The paste of the following herbs is cooked by mixing it with cow’s urine and a sweet is prepared –
– grapes,
– Patola
– Bida
– Vetrakarira
– nimba
– Murwa
– Abhay
– Aqsa
– Badara
– Amalaka
– rainbow tree
– karanj seeds
– dry tree
Mustadi Leha
A paste of the following ingredients is mixed with sheep’s urine to make a sweet and is given to drink –
– musta
– covenant
– Trikatuk
– Haridra
– Daruharidra
– colorful
– lewdness
– Nirdahni (Chitraka)
Pathadi Leha
Powder of the following ingredients is boiled with elephant urine, cooked and drunk to make sweets –
– read
– tuga
– Ativisha and
– Rajni
Mandukaparnyadi Leha
A paste of the following ingredients should be cooked with buffalo urine and given as a dessert –
– padukparni
– Arka
– Amrita
– Langalka
Whoever consumes any one of the sweets mentioned above will get relief from the following diseases –
– stomach tumors
– anorexia
– asthma
– throat diseases
– heart diseases
Satmya Swadesharchitaan Vividhanscha Bhakshyaan Panani Moolfalshaadvarayogaan.
अद्यद्रसंश्च भिन्नां विद्धायः पृकरैभुंजित चापी लघुरुक्ष्मानः सुखानी ||12||
अस्ताफान्म विधिवात्र विरेचनां च कुर्यानम्रुदुनी शिराश्च विरेचनानि |13|
The methods of taking food in case of Aurochka are given below –
– Consumption of a variety of habitual foods, beverages, spices and salads
– Food prepared from roots and fruits in one’s own country
– Eating meat soup with
– A variety of easily digestible foods
– Food that is dry and pleasing to the heart
The following treatments are also beneficial in this disease and should be given in a mild manner –
– decoction enema
– catharsis
– clear head
त्रैन्युषानानी राजनीत्रिफलायुतानी अकर्तिक्लिकानी यावशुकविमिश्रितानी ||13||
क्षौद्रायुतानी वितरेंमुख्बोधनर्थ Manyani tiktkatukani f pharmaceutics |
To stimulate the tongue, the following powder should be licked with honey –
– Tryasana – Trikatu – Pippali, Marich and Shunti
– Rajni
– Triphala – Haritaki, Bibhitaki and Amalaki
– Yavasuka/Yavaksara
Similarly, powder of herbs having bitter or pungent taste should be made and licked with honey.
मुस्तादिराजतरुवर्गदाशंगसिद्धैः क्वाथार्ज्येनम्धुयुतार्विविद्धाइश्च लेहः ||14||
वृत्रासवार्गुद्क्रिताश्च तहत्वरिष्ताः क्षरासवाइश्च मधुमाध्वतुल्यगंधैः |15|
The cure for this disease shall be prepared and administered by mixing it with the honey or confectionary mentioned below
– Decoction of mustadi gana herbs,
– Rajtaru (Ragvadhadi Gana) and
– dashang (decimal)
The following mentioned below should also be administered –
– Fermented infusion prepared using urine or jaggery
– Fermented decoctions and infusions prepared from alkali, fermented infusion made from urine or molasses which has the odor of honey or wine

Kapha and vata are reduced by the administration of these remedies. After this, treatment should be done to increase gastric fire.

दैन्यं गाते मनसी बोधनमत्र शास्तं यद्यत प्रियं तदुपसेव्यमरोचे तु ||17||
Many times it becomes unpleasant due to not getting the desired thing and also due to fear or grief. All these reasons will lead to loss of digestive fire and strength. These patients will be given the option of those desired things. Alternatively, when such options cannot be obtained, the patient should be consoled by telling stories from mythology. When the patient’s mind becomes calm with the methods mentioned above, the patient should be encouraged to use other things as per his wish.

In this way, the fifty-seventh chapter named Arokaka Pratisedha ends in the northern part of Sushruta Samhita.
